CONCLUSIONS
Possible drug interactions were identified in one third of the hospitalized patients in the Internal Medicine, Gerontology and Psychiatry wards, being the latter with highest rate of AR.
The most frequent drug interactions were due to pharmacodynamics as well as the most dangerous from the clinical point of view.
