(Institute for High Frequency Engineering of the Ruhr-University, Germany) Abstract To visualize epidermal effects of different locally applied ointments and creams by means of high-frequency-sonography, it is necessary to use frequencies well above 20 MHz. We studied the effects of vaseline, of an oil-in-water-emulsion, of a water-in-oil-emulsion and of a 10 % urea in oil-in-water-emulsion on the finger pads of 10 patients by using a 100 MHz ultrasound unit (axial resolution 9 µm, lateral resolution 27 µm). The test sites were evaluated sonographically after an occlusion period of 30, 60, 120, 180, and 240 minutes. The water content and the additives (e.g. urea) had a great influence on the sonographic image. All images were evaluated by using image analysis. We observed a significant thickening (p < 0.05) of the stratum corneum on the finger pads, a density alteration of stratum corneum and a reduction of the echogenicity of the skin entry echo. We conclude, that the 100 MHz sonography is well suited to evaluate the epidermal effects of externals in follow-up studies.
