I John P.Evans, having been appointed, pursuant to section 10.1 of the Pension Benefits Act Regulations, R.R.O. 1990, c.909, by an order of the Ontario Superior Court of Justice dated ____ to represent any person in receipt of a pension payable from (i) the Contributory Pension Plan for Salaried Employees of McMaster University including McMaster Divinity College or  (ii) the Contributory Pension Plan for Salaried Employees of McMaster University including McMaster Divinity College 2000 (“Plan 2000”), who has neither consented nor objected to the distribution of surplus contemplated by the Surplus Sharing Settlement Agreement entered into as of May 31, 2001 among McMaster University, the McMaster University Faculty Association, the McMaster University Staff Association, the Management Group (McMaster), the McMaster University Clinical Faculty Association, the McMaster University Retirees Association, McMaster Employees’ Pension Surplus  Committee and Koskie Minsky (on its own behalf and on behalf of all persons entitled to benefits under the Plan at any time between July 1, 2000 and December 31, 2000, inclusive who retained Koskie Minsky to execute the Surplus Sharing Settlement Agreement on their behalf),  do hereby consent and agree on behalf of all of the persons I have been appointed to represent to the distribution of surplus from Plan 2000 contemplated by said Surplus Sharing Settlement Agreement.
