Form 2151 in your Pension Surplus Kits

Hi all,

There have been a number of questions about one of the forms in the pension surplus package.  The form is 2151 from the Canada Customs and Revenue Agency (CCRA) and is to be filled in and sent to McMaster if money is to be transferred to an RRSP or RRIF.  This is a government form and to the best of our understanding needs to be completed in the following manner.

First, the individual fills out AREA I parts B, C, and D (not Part A of Area I) and the name and address (and SIN) information.

In Part B, you need to tick the appropriate box and fill in the CCRA's Registered Pension Plan (RPP) registration number which is #0215400.  This appears on T4 slips for Active members but is apparently not shown on the T4A slips for pension income.  Retirees would have no way of finding this out so that is part of the reason for this memo.  McMaster Human Resources will fill it out if it is left blank but it would be quicker if you would supply the number (#0215400).  Underneath the blank line for the CCRA RPP number, is another blank asking for Plan Number.  In our case, the two are the same, so this line can be left blank.

In Part C, you should tick the second box and indicate the $ amount to be transferred.

In Part D, you tick one of the first 2 boxes (either RRSP or RRIF) and indicate the plan number for your RRSP or RRIF.  Please be very clear about it and the "name of plan" since the funds need to be transferred directly.  Also in Part D just above the signature line is a line for the address of your financial institution (Transferee's Address).  Make
sure you complete this with a mailing address -- postal code and all.

Finally, in Part D you should add the date and your signature at the end of Area I.

The University will fill out Area II.

The form along with the transfer will go to the institution that handles your RRSP or RRIF and they will fill in the last section -- Area III and should return a copy to you.
